We are working towards more out of hours evening appointments, through Enhanced Access. 

Our practices have been working together to create a service that meets the needs of patients across Thornton and Poulton.

Evening and weekend appointments will be available through the four Wyre practices, Beechwood Surgery, Lockwood GP Surgery, Queensway Medical Centre and The Village Practice.

If you are a patient registered at one of the practices listed above, then you will be able to contact your practice to see where the service is run from on different dates. This extension of appointments at GP surgeries will be available for patients to book routine appointments in the evening or at weekends.

Our Social Prescribers aim is to improve patients’ health, wellbeing and social welfare by connecting them to existing community services/groups and forming new ones. We receive referrals from GP practices, mental health services, social services and other 3rd party organisations.

We can accept referrals for patients who are over 18, have one or more long term health conditions, have mild to moderate mental health conditions, are vulnerable or have complex social needs.

As a team, we discuss each referral and assign patients to our individual caseloads.

We aim to contact each patient within 10 days of referral. We arrange a home visit and use this to establish the patients needs and make recommendations based on our findings.

Some patients require a small amount of input such as helping to fill out benefit forms and others require greater input over a longer period, this can involve building rapport to build a patient’s social confidence or may involve working with other teams such as the mental health practitioners.

Patients will only be discharged if their needs have been met or if their needs are too complex, in which case we will refer on to a more appropriate service.

Our goal is to improve patients social wellbeing and have patients take ownership of their wellness to the point where some pressure on GP practices can be relieved and community wellbeing improves.
